2007-2008 Verification Document

To determine which verification form to download and print please answer the following questions:

a) Were you born before January 1, 1984?

b) Are you a veteran of the U.S. Armed Forces?

c) At the beginning of the 2007-08 school year, will you be working on a master's or doctorate program (such as MA, MBA or Ph.D., graduate certificate, etc.)?

d) As of today are you married?

e) Do you have any dependents other than your children/spouse who live with you and who receive more than half of their support from you, now through June 30, 2008?

f) Are (a) both of your parents deceased, or (b) are you (or were you until age 18) a ward/dependent of the court?

  • If you answered no to all the questions, download the Dependent form PDF.
  • If you answered yes to any of the questions, download the Independent form PDF.
