To facilitate implementation of major projects involving Information and Technology Services, completion of this form is required prior to initiation of the project and development of a project plan. Completion of the form will provide information for developing project requirements and timelines and will help us successfully manage your project.

Multiple Access Request

Requests related to the creation, modification, or deletion of account access for the following services:

Provide details for your request such as full name and/or RIT Computer Account Username.
Optional field: We will make a reasonable effort to meet all requests.
Optional field: Phone number you would like to be used in case follow-up is necessary.

Web Requests

Requests related to the creation, modification, and administration of the RIT web environment.

Note: If you don't see the website listed for the work you are requesting, it may be an Application Request. Please refer to the Application Work Request Form before selecting "I can't find my option" to verify the work you are requesting is not listed there.

Lab Services Requests

Requests related to the acquisition, administration, or configuration of hardware and software for lab computers and mobile devices, as well as operational management for ITS supported labs.

Database Requests

Requests related to the creation, maintenance, operation, security, and administration of ITS supported databases. This includes databases supporting RIT Enterprise applications and approved departmental applications with the exception of PeopleSoft and Oracle Applications. ITS does not provide database services for personal/individual use or non-approved applications.
