** Symposium Registration Closed

As of May 11, 2001 192 registrations were received and confirmed by the Symposium Office. The original plan called for between 125 and 175 individuals to attend the symposium. The Symposium Organizing Committee has found it necessary to close the registration process to insure that those who have already registered can be accommodated comfortably within the NTID complex. Interested persons can be added to a waiting list, in the event cancellations are received by contacting the Symposium Office.

However, most of the post-conference workshops still have room. Contact the Symposium Office for information on space in these workshops.

For those who are unable to attend the symposium, it will be possible to obtain the papers and verbatim text captions from each presentation at the symposium web site.

The Registration Forms below should be used only for adding your name to the waiting list or for registering for Post-Conference Workshops.

  • SYMPOSIUM, WORKSHOP AND HOUSING REGISTRATION The Symposium Registration Forms will describe how to register for the Symposium, the Workshops and Housing at the Symposium.

  • REGISTRATION MATERIALS FOR DOWNLOADING
    Download, save, print the registration booklet below. Follow the instructions to complete and submit your proposal.

      • Download and Save File.
      • Use Adobe Acrobat Reader to open and print the file. If you don't have Acrobat Reader, you can obtain it free of charge via the Adobe web site (installation instructions are also provided). Get Adobe Acrobat Reader.
      • Write or type your Registration information and then mail or fax your form to the Symposium office or hotel.

  • REGISTRATION PROCEDURE To register you will need to obtain the Registration Forms, either by contacting the symposium office, or by downloading them from this site. You must complete the forms and return them to the Symposium Office. There will be no online registration. You must mail, fax or send, via regular mail your registration materials. Contact the Symposium Office with any questions.
