Mid-Quarter Grade Report
Dear Faculty:
Structured Monitoring is a fee-based program offered through the Academic Support Center (ASC). The program provides one-one-one appointments that focus on improving students' academic performance.
For all students enrolled in Structured Monitoring, we ask the faculty to complete a mid-quarter report to aid in monitoring the student’s progress. They are aware that we are contacting their faculty for information and have signed a program agreement and release of information.
