Please send original (not photocopied or faxed) documents in English with the application:
The attached sponsor form stating that you have at least $ U.S. to pay for each quarter of instruction and living expenses.
A letter from a bank on bank stationery stating that you or your sponsor have at least $ in the bank.
A non-refundable application fee by check or money order in U.S. funds for $100 plus $42 for express mail and $100 if you want airport pickup. Include an additional $200 if you want us to pre-pay the U.S. government-required SEVIS fee and send the receipt to you. You may send one check for all expenses.
An official grade transcript in English from the last school or college you attended.
