Confidentiality and Consent for Treatment

Your medical information and records are strictly confidential and are maintained separately from your academic records. The relationship between the Student Health Center providers and their student patients is considered a confidential one which could be jeopardized by routine notification of parents, Institute administration, or others of the fact or nature of treatment. By being enrolled in the Institute, it is assumed that parents and students agree that students are able to give their own consent for most medical treatment, including minor surgical procedures. Notification of others, including parents, is considered the responsibility of the student and will not be usurped unless the condition is serious or the student is unable to assume responsibility for informing others.

We encourage you to discuss your health and health care with your parents. Health care matters will be discussed with your parents and others only at your request and with your written permission, except as required by the Public Health Laws of New York State.

Medical Records

Your medical record is confidential. This record includes: the Health History Form you submitted, provider notes, lab test results, and communications from other providers sent to or requested by the SHC. Records are kept for 10 years after graduation and are then destroyed.

Release of Information

To have information from your medical record sent to others:

  • Complete and sign a Release of Information Form (available at the Student Health Center)
  • Specify the information to be released
  • Identify the recipient's name, address, phone and fax numbers
  • Pay the applicable fees as appropriate
