• Sick Leave Form
  You can use this link to submit request for assessment make-ups due to a sick leave.
   
• Registrar Forms
  This links to the Registrar's website where you can find forms such as Add/Drop, Course Withdrawal, Change of Program, Change of Name, Minor Authorization, and Transfer Credit Requests. Please complete the form and get any necessary signatures before sending it by email to your Academic Advisor. Please use this Guide to help you fill out the forms.
   
• Prerequisite Waiver Form
  Use this form to request to take a course for which you have not met all the pre- or co-requisites.  You must have a valid reason for this request and should be able to demonstrate why you feel you will be able to succeed in the course without the required background.
   
• Overload Request Form
  Use this form to request to take more than 18 credits in one semester.  Please note that you must have a cGPA of 3.0 or higher to make this request, unless you are graduating within 1 academic year.
   
• I Need This Course Form
  This form is used for students who are on the waitlist for a course to explain their need for the course or any special circumstance that should be taken into consideration.
   
• Online Course Exception Request Form
  Use this form if you are seeking approval to take an online course offered by another RIT campus in the same term that that course is being offered at RIT Dubai.
   
• Clearance Form (Leave/Withdrawal)
  Please complete this form if you are taking a Leave of Absence for a semester or more, or completely withdrawing from the university.
   
• Release of Information Form
  All students should submit this form, which is used to designate the individual(s) who you are allowing access to your confidential academic records.
   
• Letter Request Form
  Use this form to request any official letter.
