Change of Name

Scope

This policy applies to all employees.

 

Policy Statement

It is the employee's responsibility to ensure that the personal information in his or her employment records is accurate.

 

Procedures

In cases where an employee needs to change their name for any reason, the employee must complete an Employee Change of Personal Information form and present it to the Human Resources Department with a form of identification or other document showing proof of the legal name change.
