This policy applies to all employees.
It is the employee's responsibility to ensure that the personal information in their employment records is accurate.
In cases where an employee needs to change their legal name on file for any reason, the employee must complete an Employee Change of Personal Information form and present it to the Human Resources Department with a form of identification or other document showing proof of the legal name change.
