Disability Accommodation Request
This policy applies to employees and applicants for employment. The Human Resources Department works with individuals with disabilities who are employees or applicants for employment to arrange changes in the work environment or procedures that enable individuals with disabilities to experience equal employment opportunities.
Request form
To request accommodations in the workplace due to a disability, employees should speak to their supervisor or Lori Sykes, Compliance and Ethics Manager. Employees will be asked to submit a completed Medical or Disability Accommodation Request and documentation of the disability. RIT does not provide the following services:
- Mobility assistance or training
- Personal care services
- Personal adaptive equipment such as hearing aids, eye glasses and wheelchairs

Documentation
Documentation verifying the disability should be a recent letter on letterhead stationery from a qualified professional such as a physician, psychiatrist, neurologist, or psychologist who has made the diagnosis of impairment or condition and/or you are currently seeing for treatment of said condition. The documentation should include the following information:
- Diagnosis of disability
- Description of current level of functioning
- Suggestions for accommodation(s)
- Anticipated duration of the accommodation(s)

All medical and other confidential information regarding a disability will be kept confidential in accordance with applicable laws.
