Description
The In-Home Therapist (IHT), using the principals and guidelines set forth by the Children’s Behavioral Health Initiative, provides family therapy for the purpose of treating the youth’s behavioral health needs in the context of the family. The IHT will develop a treatment plan and goals, and work with a team consisting of a Therapeutic Training and Support (TT &S) professional as well as the youth and family to identify goals, supports, resources etc in order to promote and sustain treatment goals. The children and families that work with the Walden Community Services Program must have a deaf member.

The ICC, using the principals and guidelines set forth by the Children’s Behavioral Health Initiative, provides coordination of care to children with an emotional or behavioral health need on Mass Health and to their families. The ICC will also provide Care Coordination to families working with the Department of Children and Families to provide Family Support and Stabilization Services. The children and families that work with the Walden Community Services Program must have a deaf member.

IHT FUNCTIONS AND RESPONSIBILITIES:

Works with youth and family to determine medical necessity eligibility by completing required assessment tools.

Works with the family, youth and TT &S to develop treatment plan and goals.

Regularly meets with youth and family to work on treatment plan and goals.

Works from a strength based and family centered perspective.

Liaisons with collateral agencies, schools, doctors etc. as needed.

Completes required paper work in compliance with Mass Health standards.

Works well independently and as part of a team.

Demonstrates professional conduct and maintain confidentiality.

Maintains complete, accurate, and current records of all clinical services rendered.

Some lifting may be required when working with young students or students with special needs, however, TLC embraces and celebrates a spirit of inclusion and diversity, and reasonable accommodations and modifications will be made whenever possible. The diversity of our student population may require that some assignments have a heavier physical responsibility due to safety. Develops Care Plan Team with the family and the youth.

Regularly meets with team and family to establish goals and tasks.

Works from a strength based and family centered perspective.

Works as a liaison with collateral agencies, schools, doctors etc.

Completes required paper work in compliance with Mass Health standards.

Works well independently and as part of a team.

Demonstrates professional conduct and maintain confidentiality.

Maintains complete, accurate, and current records of all clinical services rendered.

**May be required to drive students and additional driving responsibilities as needed.
