CISI - Application Form

In this application, you are asked to provide information about your education, interpreting experience, and professional development.
It is important for you to review the entire application before beginning so that you can have ALL your video links, files to upload, and other information ready BEFORE you begin the application.

PLEASE NOTE the following requirements, or your application may not be considered:
- All form fields with a red asterisk * are required. If you submit the form with any remaining uncompleted required fields, the form will not submit. You must see a confirmation message to know the form has submitted.
- All text information must be provided in English.
- Do not send links to documents or videos that reside on your own computer.
REQUIRED DOCUMENTS/VIDEOS
- Curriculum Vitae (CV), (enter all in one single file):
The three (3) items below must be included in one CV file only, all items are required. All text must be in English.
Only one file is accepted so make sure you have all information before sending. Save and upload your file in PDF, doc, or docx format.- Educational Degrees
- List (in English): your educational background (College, Degree Awarded, Major, Year).
- Professional Interpreting Experience
- List (in English): your professional interpreting positions with dates of employment. Your experience must show at least five (5) years of national or international work in your national sign language.
- Interpreting Training / Workshops Attended
- List (in English) your the trainings or workshops you have attended within the last two years — include the names, locations, and dates.
- Educational Degrees
- Professional Certification
- Submit your national sign language interpreting certification (the actual certificates — not training summaries or descriptions).
- If you do not hold a national certification, you must submit recognised equivalents either: two (2) letters in English or two (2) videos in International Sign (or combination of letters/videos) — one submission should be from the interpreting community and one from the Deaf community in your country.
- Any videos must be uploaded to YouTube, set as Unlisted, and the links to the videos provided.
- Personal Statement (video)
- You must create a 3–5 minute personal statement video in International Sign, upload it to YouTube, set as Unlisted, and provide the video link.
FORM COMPLETION and SUBMISSION
- You will see a confirmation message if you have successfully submitted the form. If you do not see a confirmation message after clicking the submit button, do not close the page but scroll up and check the form for error messages concerning missing information. When you have successfully submitted the form, you will also receive a confirmation of your submission in the email address you provided.
