I. Job Summary

The Transition Coordinator assists high school staff with planning transition instruction and assisting secondary students and their families in obtaining services necessary to transition from high school to a post-secondary setting, whether for post-secondary education, employment, or long-term support.

II. Duties and Responsibilities

Provides 1:1 counseling and instruction to secondary students that is designed to meet their individual transition needs.

Develops curriculum designs and implements lesson plans and assesses student progress in conjunction with secondary teachers.

Disseminates information to staff, students and parents regarding student’s transition needs, progress and resources

Coordinates transition activities for college, career, and community readiness, included but not limited to career fair, transition fair, and/or community based activities.

Manages IEPs for assigned caseload; coordinates and facilitates IEP meetings, writes IEPs, tracks IEP goal progress, conducts transition evaluations, and communicates with secondary teachers and support staff.

Facilitates referral, application, eligibility process for students and families to service providers and/or colleges/ post secondary institutes.

Assists with alignment of transition continuum of services at the secondary level.

Selects appropriate educational transition activities for the students assigned to him/her which will meet the predetermined learner outcomes of the curriculum and the graduation standards of Minnesota.

Works collaboratively with outside organizations and ensures staff, students, and families are informed about services these organizations and community resources provide.

Demonstrates knowledge of and application of bilingual education and methodology as it relates to the education of Deaf students.

Modifies and adapts materials to meet the individual needs of each learner.

Demonstrates fluency in ASL and written English, with an understanding of the purpose of/ development of and application of L1 (ASL).

Responsible to keep an up-to-date running record on students’ high school credits, grades, and standardized assessment scores.

Track students after graduating from Metro Deaf School.

Works with and supports the District Assessment coordinator with training, administering, scheduling, and discussing with students, college and/or career/transition assessments.

Attends and participates in staff meetings, Professional Learning Community (PLC) and team meetings for staff development, committees and curriculum projects.

Attends and participates in regularly scheduled activities outside of school hours as directed by school administration.

Works with support staff, parents, lead staff and administrators in dealing with student concerns in and outside of his/her classroom.

Maintains current licensure in the state of Minnesota.

Pursues professional development activities to broaden and deepen knowledge of content and teaching skills and continually identifies professional needs and develops strategies to meet these needs.

Performs other duties as assigned by the Principal.

III. Job Outcome

Maintains a positive and professional educational environment at all times.

Educates students and families about transition services available and assists them in gathering information and making transition plans for post secondary.

Provides a safe and effective learning environment.

Develop and maintain positive relationships with all students and families, with special attention provided to linguistically and culturally diverse students and families.

Conditions of Employment:
Selected applicant must verify experience/highest degree held
Minnesota Teaching License required (with application process initiated
by July 2020)
Upon an offer of employment, background checks will be conducted

Minimum Qualifications:
BA or Master’s Degree in Deaf Education, School Counseling, or School Social Work; ability to obtain a Minnesota License required
Native-user or fluency in American Sign Language required
Knowledge of Bilingual Education
Competency with instructional strategies
Demonstrates a strong background or specific training in the field of Transition
Must be able to work with staff, students, and families of diverse linguistic and cultural backgrounds
Ability to work collaboratively with school staff and community organizations
Knowledge of IEPs and assessments with specific emphasis on the five areas of transition
Knowledge of various learning styles and strategies for diverse learners
